Community Health Sports Medicine will be hosting a Sports Physical Night at Beech Grove High School on Monday, June 2nd from 6:00–8:00 PM. 📍 Location: Enter through the Athletic Lobby behind the football field press box. 💲 Cost: $20 per physical (Payment accepted via cash, check, or card) 👥 Important: All athletes must be accompanied by an adult. 📝 To save time, please visit https://www.ihsaa.org/resources/health-well-being and print the IHSAA Physical Form. Fill out the medical history section ahead of time and bring it with you. 💻 Make sure your Final Forms account is created for Middle School and High School athletes. We will check off your physical as completed during the event. ✅ Sports physicals are available for elementary through high school athletes and will be valid for the 2025–26 school year.
